The Concerned Committees, Offices, Bureaus, State Holding (Group) Corporations, the Municipal Settlement & Administration Center for Social Insurance Funds, the District and County Labour & Social Security Bureaus, the District and County Administration Centers for Social Insurance Funds, the District and County Population & Family-Planning Committees:
In order to further facilitate maternal women receiving Maternity Living Allowances or Medical Subsidies and to simplify the claiming procedures, The Circular on the Procedures of Claiming for the Maternity Insurance Benefits is as follows, 1. After childbirth or miscarriage, the maternal woman who has met the relevant requirements is entitled to applying for the maternity insurance benefits directly in the convenient social insurance agencies at district or county level. 2. When going through the claiming procedures, the maternal woman who has met the relevant requirements shall fill in the Application Form for Maternity Insurance Benefits and provide the following documents: 1). Identity Card of the concerned woman and the bankbook in her true-name (the original copy and the photocopies); 2). The Childbirth Medical Certificate issued by Medical Institutions; 3). The Residence Booklets of the maternal woman and her spouse, Marriage Certificate or Single Child Certificate. In addition, for the maternal woman permitted to give a birth again, the confirmation documents of childbirth by the district or county Population & Family-Planning Authority shall be required. For the unemployed maternal woman, her Labour Handbook confirmed by the Unemployment Insurance Agencies shall be provided. For the maternal woman covered by the Suburban Social Insurance Scheme, the Application Form of Maternity Insurance Benefits for Suburban Social Insurance) issued by the Administration Centers for Suburban Social Insurance at district and county level where she opens the suburban social insurance account shall be provided. For the maternal woman who is not a permanent Shanghai resident, the certificate of childbirth permit by the Family-Planning Authorities of the county or town (township) government or the community office in the region of her permanent residence shall be provided with other relevant documents. 3. After childbirth or miscarriage, the maternal woman who has met the relevant requirements may authorize other person for the maternity benefits claiming procedures, and the Identity Card of the authorized person (the original copy and photocopy) and the Trust Deed shall be provided with other relevant documents. 4. The district/county Social Insurance Agencies shall strengthen the verification on applications for maternity insurance benefits, and shall timely inform the local district/county Population & Family-Planning Committees with the copies of the Childbirth Medical Certificate and the permanent residences of the maternal women. The Population & Family-Planning Committees at district and county level shall strengthen the supervision work afterwards over the claiming for maternity insurance benefits, and shall actively cooperate with the Labour & Social Security Authorities to investigate and prosecute the people involved in case that the maternal woman has violated the Family-Planning policy and falsely claim Maternity Insurance Benefits. 5. The Circular shall become effective as of May 1, 2003. “The Regulations on the Verification Procedures on Family-Planning for Maternity Insurance Benefits Claiming” (Document No (33) 2002 by Shanghai Municipal Population & Family-Planning Committee) shall be abolished simultaneously.
